Before you take your test and mail your sample, please complete the form below. This is required for tracking the progress of your test through the testing, results, and consultation steps.

• Your kit ID# is on a sticker on the back of your test box. Be careful to input that accurately.

• If you purchased more than one at-home test, you'll need to complete and submit this form for each test.

• We suggest taking a picture of your ID# sticker, for safe keeping. Hang on to your test box until you receive results.
